Hydrochlorothiazide Mechanism of Action.
Hydrochlorothiazide increase the urine volume and thereby decrease cation concentration in blood, extracellular fluid volume, plasma volume, followed by decrease in peripheral resistance and normalization of cardiac output. These actions produce decrease in peripheral resistance and lower the blood pressure. The efficacy of these agents as an antihypertensive is not directly related to diuretic potency, and hence blood pressure lowering may result from other mechanisms as well.
